ORIGINAL POST: Well Child Schedule (Immunization schedule)
Birth: Newborn examination by physician, Hepatitus B Vaccine (HBV)
3-5 Days: Jaundice evaluation, Examination, hearing screening, and feeding issues discussed
2 Weeks: Examination, PKU test (Bring your PKU paperwork from the hospital with you to this exam)
2 Months: Immunizations (DTaP, HIB, Polio, HBV, Prevnar), screening examination
4 Months: Immunizations (DTaP, HIB, Polio, HBV, Prevnar), screening examination
6 Months: Immunizations (DTaP, HIB, Polio, HBV, Prevnar, Influenza in winter months)
9 Months: Screening exam
12-15 Months: Screening Examination (DTaP, MMR, Chicken Pox Vaccine, Prevnar, Polio) ASQ (ages and stages questionaires)
18 Months: Screening Examination, ASQ
24 Months: Screening exam, Hepatitis A, ASQ
3 Years: Screening exam, ASQ
4 Years: Screening exam, ASQ
5 Years: Screening exam, Immunizations (DTaP, Polio, MMR) ASQ
